This is a tested and working PCI video card pulled from an IBM dual CPU server tower.  

 
The system was running Windows 2000 and when I upgraded it to Windows XP I couldn't find any drivers for this card.  So I exchanged it for a cheaper card that XP had built in drivers for.    
 
Features two VGA ports and what appears to be an S-Video port.   Graphics were great in Win 2000 with no problems.    Also has what appears to be 50 pin SCSI port.   Uses Cirrus Logic GD5440 chip, says MSMT440 Rev. B p/n 01K2013.    Drivers available on the web and it appears as if they are still downloadable direct from Intergraph by clicking here .
 
Metal piece on the end is removable.
